How to Make an Appointment
All patients seen in the clinic by the Movement Disorder Neurologists must be referred by a physician or nurse practitioner with referring privileges.
The referring physician will need to fax a consultation letter or the “Request for Services” form (below) to 204-940-8414. It is up to your physician to decide how to manage your health issues until you can see us. Your physician can call us anytime for advice.
Other neurologists in the community can also be of great assistance in the care of Movement Disorders.
The clinic resource nurse can be contacted for information as needed. If you would like to see this nurse in the clinic though, an appointment would need to be arranged in advance.
