Comprehensive Geriatric Assessment Clinic
The Comprehensive Geriatric Assessment Clinic (CGAC) provides assessment and rehabilitation services to promote healthy living for seniors in the community and reduce potentially preventable hospital admissions.
Clients attend individualized appointments and are discharged after rehabilitation is completed. Team members include a dietician, geriatrician, nurse, occupational therapist, pharmacist, physiotherapist, secretary, speech language pathologist and social worker.
